The post-operative period after a hair transplant is one of the aspects that raises the most questions among patients at Instituto Médico del Prado. Will scabs form after the procedure? What post-operative care is required? Can a hair transplant cause headaches? How should I wash my hair after the procedure? In this article, we answer these and many other common questions about the recovery process after your hair transplant.

Is the recovery after a hair transplant uncomfortable?

No. As hair transplantation is a minimally invasive procedure, most patients experience very little discomfort during the days following surgery. The post-operative instructions are primarily aimed at protecting the newly implanted grafts and ensuring the best possible outcome.

Some patients do experience mild facial swelling after the procedure, particularly on the forehead. This is completely normal and simply indicates that the local anaesthetic is naturally wearing off. Although it may be slightly noticeable, it is generally not painful.

Is post-operative care necessary after a hair transplant?

Yes. In fact, proper aftercare is essential for achieving the best possible results.

In the donor area (the back of the scalp), you should remove the bandage 24 hours after the procedure and apply Aquaphor ointment once a day.

In the recipient area—usually the frontal scalp—it is extremely important to keep the skin constantly hydrated. Your doctor will provide enough sulphur thermal water for this purpose, as it should generally be applied every 30 to 45 minutes. If you experience itching or burning sensations, increase the frequency of hydration.

We have also found that aloe vera creams are particularly effective in relieving itching in the recipient area.

The most important recommendation during the recovery period is to rest, preferably at home, to avoid any accidents that could damage the newly implanted grafts. This includes avoiding bumps or excessive sweating, as both could compromise the growth of the transplanted hair. In severe cases, this could even make a second procedure necessary.

How should I wash my hair after a hair transplant?

This is one of the most important aspects of post-operative care, as incorrect washing may damage the implanted grafts.

During the first week, it is essential to use the shampoo recommended by your doctor, but it should never be applied directly onto the scalp. Instead, create a gentle foam and carefully dab it onto the transplanted area without rubbing.

Your doctor will advise you when and how you can resume normal hair washing. This depends on your individual healing process, which is why it is important that the same doctor who performed your procedure also monitors your recovery.

Will scabs form after a hair transplant?

Yes. The formation of scabs after a hair transplant is completely normal and actually a positive sign of healing. They usually begin to appear during the first few days after surgery, and your doctor and clinical team will explain how and when to remove them safely.

Scabs indicate that the skin is healing properly after the tiny incisions made during the procedure.

It is also very common for the scabs to fall off naturally, sometimes with small hairs attached. There is no reason to worry—this is perfectly normal. If the transplant has been performed correctly, the follicle itself remains intact beneath the skin, and the hair will grow back after the shock loss phase.

How long does recovery after a hair transplant take?

As you can see, the recovery process is straightforward but essential to ensure the best possible outcome. For this reason, Instituto Médico del Prado generally recommends taking five to six days away from normal work and social activities after the procedure.

If returning to work sooner is unavoidable, Dr. Espinosa Custodio recommends waiting at least 48 hours.

Strictly speaking, however, the hair transplant recovery process lasts up to one year, as the transplant alone does not complete the treatment. Several follow-up appointments and complementary medical hair treatments are often necessary to achieve the desired final result. Without them, hair thickening may be suboptimal and overall density may not meet expectations.

Do I need to take medication after a hair transplant?

Only the medications typically prescribed after a minor surgical procedure, such as antibiotics, anti-inflammatory medication and stomach protectors. Your doctor will prescribe these if necessary.

Long-term oral hair-loss medication is a separate matter. A hair transplant only partially addresses androgenetic alopecia because the underlying condition is progressive and has no cure. However, its progression can be slowed with medications such as finasteride or minoxidil. For this reason, these medications are often recommended during the first year following surgery.

How should I sleep after the procedure?

The recommended sleeping position is semi-upright, avoiding any contact between the pillow and the transplanted grafts. Using a travel neck pillow can make this position much more comfortable.

Are there any restrictions I should be aware of?

It is advisable not to drive immediately after the procedure. You should also eat a light meal, avoid alcohol and smoking, and wear loose-fitting clothing to prevent any friction against the transplanted area.

All of these post-operative recommendations should be clearly explained by your hair transplant specialist before the procedure to help ensure optimal, natural-looking results. At Instituto Médico del Prado, we provide every patient with a comprehensive pre-operative and post-operative guide, as well as a dedicated hair restoration care kit to support recovery.
